What does it mean for a healthcare provider to be out-of-network?

Healthcare providers who are out-of-network are usually not covered by your health insurance policy. You will usually have to pay out-of-pocket for out-of-network providers. This payment is much higher than it would be if you had gone to an in-network provider. However, if your insurance company is a PPO or a POS, depending on their policy, the insurance would pay for some of the costs of out-of-network providers.
